Psychiatric Advance Directive Information

Talk to your doctor

A mental health status examination is required to complete your PAD. Your primary care physician or a licensed psychologist. Call and schedule an appointment to go over your wishes and have the exam completed.

Update your plan

If at anytime your wishes change you may create a new PAD. You will need to complete the Mental Health Status examination again. If unchanged, PADs remain in effect for 5 years.

Discuss with your care team

It is important to make those closest to you familiar with your wishes. This includes your therapist. They are here to help. Please make sure they receive a copy of your PAD and that you discuss it together.
